This form is used for all breeds with the exception of Sighthounds, which have their own application form for double championship. Please use this form when applying for an International Beauty Champion title for a dog with working trial merits. A minimum of two CACIBs are required for the International Beauty Champion title. Please use this form when applying for an International Beauty Champion title for a dog without working trial merits. For dogs with no working trial merits, a minimum of four CACIBs are required for the International Beauty Champion title. If your place of residence is not in Finland, please send the form to the national kennel club in your country.

A dog owner can request that a reserve CACIB is changed to a CACIB by using this form. The request can be made if the owner of the dog awarded with a CACIB announces that his or her dog does not need the CACIB.

A dog owner can request that a reserve Nordic Show Certificate is changed to a Nordic Show Certificate by using this form. The request can be made if the owner of the dog awarded with a Nordic Show Certificate announces that his or her dog does not need the Nordic Show Certificate. This form is to be used when filing a complaint of the judging in a trial, competition, mental test, or dog show.

This form is used to apply for an international junior or veteran champion title.
